Driving habits questions were taken from the Driving Habits Questionnaire (Owsley, C., Stalvey, B., Wells, J., Sloane, M.E. (1999) Older drivers and cataract: Driving habits and crash risk. Journal of Gerontology A: Biological Sciences Medical Sciences 54A: M203-M211.)

As the data we collected includes many items containing personally identifiable and confidential information, the raw data will not be available on this data sharing website; however the results of our survey are summarized in the paper.
